Round-Up Debit Card Enrollment

Round-Up Debit Card Enrollment

You can use this form to enroll your debit card for PCM Credit Union's Round-Up program and round up each of your debit card purchases into one of your savings accounts to help you save money. Information on the form is required to complete enrollment.

Cardholder Name

Cardholder First Name
Cardholder Last Name
The account number your debit card is associated with.
Last 4 digits of the debit card you want to enroll in Round-Up (each card on account will need to be enrolled separately)
Round-Up Enrollment
A Round-Up request must be submitted for each card holder requesting to Round-up debit card purchases. Each card holder can select a different savings account to Round-Up into.
Example 01 Savings, 02 Savings, 03 Savings, Etc. (Round-Up cannot be deposited into HSA Savings, IRAs, CDs or Escrow Savings Accounts)
