Stop Payment and Other Checking Account Forms

To notify us that your checks have been lost or stolen please call the credit union immediately at (920) 499-2831 or toll free at (877) 373-4023.

A stop payment is a request to cancel or prevent the payment of a check that you’ve already written but don’t want to be cashed or processed.

For security reasons, we require a signature to initiate a stop payment. Please print and complete this stop payment form, then drop it off at any PCMCU location at your earliest convenience. You may also contact us at (920) 499-2831 to receive a stop payment form for electronic signature.

There is a nominal fee to cover the paperwork involved in this request and it will be automatically deducted from your checking account upon receipt of this form. Please contact us for current fee information.
